Umrao hospitals Mira road has once again proved its existence as a keystone of Health care sector in North Mumbai by doing a successful Organ retrieval surgery on a 39 year old brain dead patient. The internationally qualified team of doctors with skilled nursing staff could successfully retrieve her kidneys, liver and cornea.
Mrs.Ashiwini Naik (name changed), 39 years, was being treated for Stroke (cerebrovascular accident) at private hospital in Mira road where her condition deteriorated and she was transferred to Umrao hospitals – the only multispecialty tertiary care hospital. On admission she was not responsive, with CT scan suggestive of massive infarct in right middle cerebral artery with mid line shift which is a life threatening condition. To strive her life doctors at Umrao hospitals decided to operate her however her condition could not improve and she was declared brain dead.
The medical team at Umrao hospitals convenience her relatives for organ donation so that the other valuable lives could be saved. Organ retrieval was started on 2nd July at 5.30 am and both Kidney, liver and cornea were removed as per the zonal transplant coordination center guidelines. This can give new lease of life to at least 3 patients awaiting renal and liver transplant and sight to at least 2 patients awaiting corneal transplant.
Considering the fact that there is a long and ever increasing list of patients waiting for the organs to be transplanted and there is less awareness of organ donation in India as compared to western countries the step taken by Umrao hospitals is considered as a milestone. Such successful surgery was made possible at Umrao hospitals only because of its state-of-art infrastructure and ultra modern operation theatres with laminar air flow system to achieve zero bacterial count and dedicated team of medical experts.
31 Years Male Mr.Abhishek Agrawal, admitted in Umrao Hospital on 24th Feb 2014 with chief complaints of Fever, Cough & Dypsnoea. X ray chest showing B/L infiltrates with consolidation signs s/o early ARDS.Initially patient was kept on oxygen by mask which in course of time with worsening of lung condition changed to NIV Bi-Pap support. ARDS worsened with rise in sepsis, patient developed respiratory distress. Patient was Intubated & put on ventilator. With the suspicion of H1N1- swine flu, patient was started on T. Tamiflu from admission. Condition of the patient worsened with development of sepsis. Ionotropic supports for maintenance of BP, Ventilator for respiratory support as well as antibiotics.
Expert opinion from Dr. Prabhudesai suggested the option of ECMO. With deteriorating patient’s condition relatives also accepted the option. With Dr.Venkat Goyal & his team’s expert opinion ECMO was started on 28th Feb 2014.
Broncho scope & throat swab confirmed the diagnosis of H1N1.
Unfortunately patient had developed pseudo aneurysm in the groin- Rt. Femoral region which required surgical bedside exploration.
Eventually with all supportive treatment, proper management with timely intervention & early decisions patient improved gradually as evidenced by radiological studies, arterial blood gases & clinically.
Patient was supported on ECMO for almost 9 days. Patient stabilized with all the efforts & management. Currently patient is off ventilatory support, in deluxe room, out of ICU –weaning mode, conscious , responding, stable hemodynamically.Surely he will get recover completely in 1 or 2 days, he will require good nursing care & aggressive physiotherapy as well as good counseling with Physical & Mental support.
It was a miraculous effort of all the experts-doctors, nursing staff & support staff.
A 69 years old patient,Mr.P.G.Shinde,suffering from Hypertension with Chronic Kidney Disease ( CKD ),Nephrosclerosis,was brought to Umrao Hospitals with accelerated blood pressure and was admitted under Dr.Pradeep Vyas,Interventional Cardiologist. His CAG with peripheral angio was suggestive of 100 % occluded Right Internal Carotid Artery.
On the day of 29th May Mr.P.G.Shinde was admitted at Umrao Hospitals with the accelerated blood pressure of 220 / 110 mm of Hg.His MRA brain showed Right Internal Carotid Artery occlusion at its origin.Left Internal Carotid Artery with focal narrowing close to origin with good distal flow.MRA renal was suggestive of 50 % focal narrowing in Right Renal Artery. With Herculean efforts of Dr.Nitin Dange, the eminent Neurosurgeon performed his DSA ( Digital Subtraction Angiography ) with Carotid stenting / Carotid Angioplasty.Mr.Shinde was discharged within a weeks time and leading a healthy life ahead.
